Intake Process
DBT Eastside understands that time is of the essence, thus we strive to maintain a straight-forward and efficient intake process. If you are interested in scheduling an intake to enroll in a DBT Skills Group, Gold Standard, or Individual Therapy, please review the information below as it covers key information all clients should be aware of prior to engagement.
Once you have reviewed the information below, please call us at 425.615.0605.
IMPORTANT NOTE: An intake assessment conducted by DBT Eastside is not a guarantee of engagement of services. If our screening indicates that another form of treatment or level of care is more appropriate and we are not able to serve you, we will communicate our recommendations for your treatment needs

Intake is a 120-minute appointment during which a specially-trained therapist (Intake Coordinator) will go over our program in more detail. Our Intake Coordinator will discuss the clinical aspects of DBT treatment and answer any clinical questions you may have. Finally, the Intake Coordinator will conduct a private interview with you to discuss background and treatment goals in order to recommend a course of treatment. Should you wish to engage in any level of our services, our Intake Coordinator will also review enrollment forms as needed.
Prospective clients who attend an intake are under no obligation to commit to any additional services at the time of the appointment.

Please call 425-615-0605 for a free phone screening and to speak to an intake specialist. They will ask you some screening questions after which you will receive a link via email to a secured client portal with all required forms. Please allow at least an hour or more to complete these forms. Once forms are complete and assessments reviewed, we will contact you for either further questions/assessment or to schedule your intake appointment.
The intake process takes 60-120 minutes, depending on questions and material to review. Your intake specialist will review program information, contract details, relevant assessments, and information from your intake form with you. Treatment recommendations are typically made during the intake, barring further necessary assessment. Please bring all of your questions and concerns to your intake appointment!
Please note that currently our intake process is online via Zoom due to Covid-19 and state requirements. You will be sent a link to your meeting the day before it is scheduled.
-
Failure to attend a scheduled Intake appointment without providing notice (No Show) or providing less than 48 hours notice (Late Cancellation) is very disruptive to our Intake Coordinator’s scheduling efforts and may prevent us from serving other requests as important as yours. A $100 fee will be incurred for missing your Intake session under these conditions. Please be prepared to provide credit card information at time of scheduling. Your card will not be charged unless the fee is incurred.
We do understand that situations may arise that cause late cancellations and no shows. The $100 fee will be waived if you choose to reschedule your appointment.

Please do not use navigation programs to find our location. There is an error in them that sends you to a completely different area of Bellevue. Please use the directions below:
We are located in the Bellefield Office Park (98004). Light Rail construction has limited access to the office park so please use the entrance off of SE 8th street, between 112th & I-405.
From I-405
• Take the SE 8th Exit, WEST bound
• Bellefield Park will be on your LEFT
From I-90
• Take the Bellevue Way exit, stay in the right lane (construction prevents you from changing lanes after the first stoplight)
• Turn RIGHT on 112th
• Turn RIGHT on SE 8th
• Bellevue Park will be on your RIGHT
Once you have entered Bellefield from SE 8th, proceed over 4 speed bumps and the Cedar Building will be on your RIGHT; same parking lot as the courthouse. If you cross over a small bridge and meet a Y in the road, you have traveled too far.

If you decide to engage in services with DBT Eastside, our Intake Coordinator will work on assigning you to a therapist on our team. Once a therapist has been assigned, they will contact you within 5 to 7 days to schedule your first appointment. On occasion, therapist assignments may be delayed, however, we will do our best to to assign you as soon as possible.
